Insurance

We are happy to verify your insurance benefits when you contact our office.  Please click and complete our Insurance Verification Form and we will follow-up with you very quickly.  Cash, check and all major credit cards accepted for payment.

If you do not show up for your scheduled appointment, and you have not notified us at least 24 business hours in advance, you will be required to pay the full cost of the session before scheduling any future sessions.
